The Settlement Administrator has been engaged by the Office of the New York State Attorney General (“OAG”) to assist it in implementing the settlement in the OAG’s Investigation of NAE Edison LLC, DBA Edison Home Health Care; Assistcare Home Health Services, LLC, DBA Preferred Home Care of New York; and Samuel Aryeh Weis, Shumel B. Weiss, Elliot Segel, and Charles Blumstein.

If you wish to participate in the settlement, please submit the payment selection by February 3, 2025.

In accordance with the New York State Personal Privacy Protection Law, any personal information that you may provide, such as your name, address and/or banking information, will only be used to process your payment. Simpluris will retain your information in a secure database for only so long as is needed to process your payment and report such payment to the OAG. If you have concerns about data safety, privacy, or submitting your personal information, please contact labor.bureau@ag.ny.gov for further information.

This Assurance of Discontinuance (“AOD”) contains the OAG’s findings in connection with this investigation and the relief agreed to by the OAG, Edison and Preferred, and the Former Operators.
